South Carolina’s Most Expensive Hospital

 South Carolina has about 65–70 acute-care hospitals. Lexington Medical Center is the most expensive one for all services.

Lexington Medical Center is not a small county hospital anymore. It is a 600-bed system with nearly 80 physician practices, thousands of employees, and well over a billion dollars a year in patient revenue.

In late 2025, even Cigna said publicly that LMC is the highest-cost hospital in the area and that its rates were significantly higher than others. That is what decades of public support bought us. LMC started as a Lexington County hospital in 1971. Taxpayers built it. It still enjoys nonprofit tax exemptions and has used hundreds of millions in tax-exempt bonds. We subsidized the campus, the expansion, and the brand. Then we get billed like a luxury hospital for routine care.

Same MRI, same labs, same outpatient procedure — often two or three times what an independent clinic or imaging center charges a few miles away.

A cardiac loop-monitor removal that a private cardiologist does for a few hundred dollars has been billed in the tens of thousands on a hospital campus. That is not “quality.” That is a monopoly markup on a building the public already paid for. Look at your property tax bill, your insurance premium, and your last hospital statement. You paid three times: once as a taxpayer, once as a premium payer, and again at the window. If a hospital wants to be the most expensive shop in the state, it should compete on price in the open.

It should not do it while sitting on tax breaks and public capital and then telling families they have no other choice. Shop the cash price. Ask for the CPT code. Compare LMC’s list to an independent facility before you schedule anything that is not an emergency. And ask County Council a simple question: after 50 years of subsidy, why are Lexington families still paying the highest prices in the neighborhood?
